How to apply :-
The application must be made in a special form that can be obtained from
Bahagian Pasar Awam, Jabatan Kesihatan di
Pejabat Cawangan Kompleks Pasar Besar Jalan Meru, 41050 Klang
with a price of RM1.00 and a processing fee of RM10.00.
The completed application should be sent to the above address for processing.
PREMISES APPLICATION GUIDELINES :-
1) Only applicants who complete the premises application form will be called for an interview
2) Applications that are not filled out completely WILL NOT BE ACCEPTED
3) Premises application qualification must be a MALAYSIAN CITIZEN and born in Malaysia
4) Applicants must be 18 years old and not more than 60 years old on the closing date of the advertisement
5) If the applicant applies in a location where there is NO VACATION/ NOT ADVERTISED then
the application will be REJECTED without an interview
6) For applicants who were NOT BORN IN MALAYSIA, you are requested to include a copy
"BIRTH LETTER".
